Tattoo Consent Form

Do you have any skin conditions in the area being tattooed that may affect the tattoo?

I acknowledge that it is not reasonably possible for the representatives and employees of True Tattoo to determine whether I might have an allergic reaction to the pigments or processes used to carry out my tattoo, and agree to accept the risk that such a reaction is possible. 

I understand that a new Tattoo is susceptible to infection until fully healed and that the proper aftercare of the tattoo is my responsibility on leaving the studio. I confirm that I have been given aftercare advise in writing and that it has been explained to me and I understand it. I understand that notwithstanding the hygienic conditions and sterile instruments and techniques used by this tattoo establishment, there are associated risks with tattooing, which include infection, scarring, allergic reactions, localised swelling and ink migration. 

I understand that any touch ups required after my tattoo that occur as a result of failure to follow the aftercare advise given to me will incur an additional cost

I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo. I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo.

To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my wellbeing as a direct or indirect result of my decision to have a tattoo

I acknowledge I am over the age of eighteen and that I have truthfully represented to my tattoo artist that the obtaining of the tattoo is my choice alone. 

I consent to the application of the tattoo and to any actions or conduct of the representatives or employees to the Tattoo shop reasonably necessary to perform the tattoo procedure.

I understand and accept that a tattoos are created by hand and that the appearance of the artwork will differ from the tattooed procedure.

I agree to release and forever discharge and hold harmless True Tattoo and it’s independent contractors from any and all claims, damages and legal actions arising from or in any way connected to my tattoo and the procedures and conduct used to apply my tattoo.

Do you suffer from any of the following conditions:

I am not pregnant or nursing and that if I do have any conditions that I feel may affect the healing of my tattoo I will advise my tattoo artist.

I confirm that I am not under the influence of drugs or alcohol.

I agree to allow any photographs taken of my tattoo to be used by the artist for media and advertising. 

I confirm that I have discussed my tattoo design with the artist, I have approved the artwork and would like the design to be applied as a tattoo.
